Insurance Coverage

MAP can contact insurers to request coverage and benefits information for a specific medication.

The Merck Co-pay Assistance Program

The Merck Co-pay Assistance Program offers assistance to eligible, privately insured patients who need help affording their medication. For product-specific
information, please visit your medication website.

Co-pay assistance from the Merck Co-pay Assistance Program is not insurance. Restrictions apply. See the Terms and Conditions for your medication.
Patient and health care professional must submit all required information. Please see the enrollment form on your medication website for details.

Independent Assistance Foundations

You may also be able to get help from an independent assistance foundation. Independent assistance foundations may be able to provide support if
you do not qualify for other forms of financial support. Each foundation has its own eligibility criteria and application process and is independent from
Merck; there is no guarantee a patient will receive any type of assistance from these organizations.

MAP representatives can refer you to the Merck Patient Assistance Program for eligibility determination (provided through the Merck Patient
Assistance Program, Inc.).

Representatives are available to answer your questions Monday through Friday from 8AM8PM ET.

Getting started is simple

Work with your health care provider to download and complete the enrollment form for your medication,
OR complete the electronic patient enrollment form if available.
Print and fax the completed downloadable form, OR submit the electronic form online if available.
A program representative will contact you and your health care provider's office.
